BOSTON (WBZ NewsRadio) — Department of Labor investigators found a New York-based firm may have been able to prevent the August death of a Logan Airport forklift operator, according to a press release.

The worker fell from the forklift he was operating and it fatally struck him, according to the press release.

According to the Occupational Safety and Health Administration, Oxford Airport Technical Services was the company that hired the worker. The company allegedly left a damaged forklift in service, did not properly train any of its forklift operators and did not ensure they wore seatbelts.

OSHA issued the company four "serious" citations and proposed $46,096 in fines, according to the press release.

“Every workplace fatality is tragic, especially when there are well-known safety measures that could have prevented the loss of a person’s life,” said OSHA Area Director James Mulligan in the press release. “In this case, the employer failed to train and certify their forklift operators on critical safety requirements. It’s simply inexcusable.”

Oxford Airport Technical Services is based out of Rochester, N.Y.
